2017 Agape Youth Choir Trip Information

WE WERE INVITED TO PARTICIPATE AT THE AGAPE YOUTH CHOIR THAT WILL BE TAKING PLACE IN NOVEMBER. PLEASE NOTE THAT WE ARE GOING TO REPRESENT OUR CHURCH, SO IF YOU PLAN ON NOT PARTICIPATING OR GOOFING AROUND, PLEASE STAY HOME!!! PLEASE REVIEW THE INFORMATION LISTED BELOW.

 

ALL THOSE PARTICIPATING MUST SIGN A LIABILITY WAIVER FORM (SEE BELOW & ATTACHED).

SIGN-UP INSTRUCTIONS: THERE ARE 10 ROOMS RESERVED UNDER THIS RATE, TO SECURE YOUR ROOM DO THE FOLLOWING:

  • GET 4 PEOPLE TOGETHER IN YOUR ROOM
  • DESIGNATE ONE PERSON WHO WILL BE RESPONSIBLE FOR THE ROOM
  • HAVE THE DESIGNATED PERSON SEND AN EMAIL TO JOEL HERMAN AT JOEL@FRPCDETROIT.COM WITH THE NAMES OF THE 4 PEOPLE IN THE ROOM
  • DEADLINE TO SIGN-UP: FRIDAY, NOVEMBER 3RD
